In 2010, a fatal explosion and fire took place at the Tesoro refinery in Anacortes, Washington. Due to the High Temperature Hydrogen Attack (HTHA), a nearly 40-year-old heat exchanger ruptured, exploding and fatally injuring 7 workers, making it the largest loss of life at the U.S. refinery since 2005.
